Rexdale Community Health Centre

Rexdale CHC serves individuals and families living in the area bounded to the north by Steeles Avenue, to the south by Dixon Road, to the west by Highway 427 and to the East by the Humber River.

Rexdale Community Health Centre delivers integrated, coordinated clinical services through a team of physicians, nurse practitioners, nurses, dietitians, social workers, a physiotherapist and a chiropodist. Our multi-disciplinary team addresses the health needs of individuals and families at all ages and stages of their lives. In collaboration with the City of Toronto’s Public Health department, Rexdale CHC also offers dental services for children, students and seniors.
