The Trans Health Program at Centretown Community Health Centre supports trans and gender diverse individuals ages 17 and up in Ottawa and the surrounding region. We offer hormone initiation, surgical referrals, counseling, and health system navigation. We have a gender-affirming and informed consent approach, which meets the standards outlined in the Sherbourne Health Guidelines for Gender-Affirming Primary Care with Trans and Non-Binary Patients.

Community Care Team is an outreach team of health care professionals from Health for All Family Health Team. We provide free services for individuals within Eastern York Region. Our Team includes: Nurse Practitioner, Social Worker, Dietician, Case Manager, Pharmacist and Health Promotor.

The program provides individual and group education on diabetes prevention and management with a nurse and dietitian.

Experienced and compassionate LGBTQ2S+ friendly, trauma-informed, weight inclusive nutritional counselling for disordered eating/eating disorders, problematic relationships with food, mental health concerns, digestive concerns. As an anti-diet dietitian, I do not provide weight loss counselling/diets. No referral required. Counselling is temporarily being conducted virtually.

The Gender Affirming Care Program was started in 2022 by nurse practitioner Joanna (she/her). She provides hormone therapy for individuals 16 and over and will see younger individuals in consultation and assist with providing appropriate resources. A social worker on our team is also available to provide psychosocial support with gender identity and gender transition. Services are provided in Caledonia, Dunnville, and Hagersville.
