The Lakeview Family Health Team (LFHT) is one of 200 Family Health Teams across Ontario, which opened as part of an Ontario Ministry of Health and Long Term Care initiative aimed at improving access to primary health care professionals in Ontario. The LFHT is composed of an inter-disciplinary team of physicians and other health professionals who work collaboratively to provide patient-centered and comprehensive primary care to residents in the communities of Colborne, Brighton, Quinte West, and the surrounding areas. Our team includes physicians, nurses, nurse practitioners, a physician assistant, a dietitian, a pharmacist, a psychiatrist, and a social worker.
