The Gender Affirming Care Program was started in 2022 by nurse practitioner Joanna (she/her). She provides hormone therapy for individuals 16 and over and will see younger individuals in consultation and assist with providing appropriate resources. A social worker on our team is also available to provide psychosocial support with gender identity and gender transition. Services are provided in Caledonia, Dunnville, and Hagersville. We are currently providing services to patients that have a family doctor within the Haldimand Family Health Team.
