Our Primary Care team consists of Physicians, Nurse Practitioners and Registered Nurses.  Clients see the most appropriate provider for the health issues and concerns.

In general an appointment with a Physician or Nurse Practitioner is a half an hour in length. Services include  diagnosis, treatment, education, referral and follow-up on health issues. Some of the specific services are complete health check-ups, well-baby checks, and immunizations. These are provided on-site at 44 Francis St. South to rostered clients only. Appointments are required.

Our Allied Health team includes:

  • a Registered Dietitian who works with clients to support healthy eating for a variety of dietary needs including chronic disease, heart health, food budgeting, healthy weight, growth and development of children and other dietary requirements.
  • Three Counsellors who offer supportive counselling for individuals (ages 4 and up), couples, families and groups.  Our counselors work to build upon people’s strengths in order to meet the challenges of life and problem solve the many issues that people may be facing.
  • A Chiropodist who specializes in the care of the foot.  This service focuses on the assessment of the foot and the treatment and prevention of disease or disorders of the foot.

Appointments are required for our Allied Health Team services.

Our Community Health Worker/Advocate provides individual advocacy for clients who need support/assistance in dealing with community agencies or who need information about community services and supports

KDCHC also runs many programs including: managing chronic pain, healthy eating, seniors programming and more.
