Get directionsAs a patient in our academic Family Health Team, you will receive excellent care that reflects the values and mission of St. Michael’s Hospital. Your care will be delivered by our skilled interprofessional team and will be centered on your needs. The Department of Family and Community Medicine is dedicated to improving care both in and out of the clinic through quality improvement and research programs. As part of our affiliation with the University of Toronto, we are a training site for students from medicine, chiropractic care, psychology, nursing, dietetics and pharmacy.
